Patient Care Manager, Hospice, Direct Hire, Full Time Days

Provenir Healthcare is actively recruiting a Hospice Patient Care Manager in Dallas, TX. In this role, you will be responsible for:

  • Supervision, orientation, mentoring, developing and ongoing evaluations of all patient care staff
  • Assure compliance with agency policies and procedures, state and federal laws, regulations, and accreditation standards
  • Participate in the manager on call rotation
  • Assist with talent acquisition to ensure adequate staffing is maintained
  • Coordinate and facilitate IDT meetings
  • Assist with quality assurance and performance improvement efforts

Candidates qualified for the Patient Care Manager position must have:

  • Associate Degree in Nursing, Bachelor’s preferred
  • RN TX License in good standing
  • Maintain CHPN or can obtain certification within 12 months of hire
  • 2 years of Hospice experience
  • 1 year of management experience
  • Valid Texas Driver’s License and Proof of Auto Insurance

Compensation and Benefits:

  • $82,000-$87,000 depending on experience
  • $5,000 sign on bonus
  • Medical, Dental, Vision
  • Short Term/Long Term Disability
