Physician Assistant or Nurse Practitioner needed in El Paso, TX (Westside) for Home Health & Supportive Care
Provenir has a National Client seeking a Physician Assistant to join their new initiative – Home & Supportive Care team led by Physicians, Physician Assistants and Nurse Practitioners.

- Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA)
- Current Licensure through the Texas Board of Nurse Examiners OR Licensed Physician Assistant by the Texas State Medical Board
- Valid DEA license or the ability to obtain one before employment
- 1+ year of clinical experience as a Nurse Practitioner or Physician Assistant
- Comfortable managing a primarily Medicare-aged patient population
- Reliable transportation
Position Highlights:
- Practice with a multi-disciplinary team of healthcare providers that includes Primary Care Physicians, RN Case Managers, social workers, and clinic staff
- Provider is accompanied in the field with Medical Assistant and Scribe in company vehicle
- Company vehicle is used for visits
- Provide care for geriatric patients whose needs are beyond what the clinic-based resources can provide but for whom hospice services are not appropriate; 5-6 patients per day.
- The model is designed to provide the most efficient, comprehensive, and proven care techniques to treat the whole patient – physically, mentally, and socially – at each visit
- Primarily manage care in the patient’s home; however, care may require an office or other facility setting depending on the needs of the patient
- Diagnose and treat both acute and chronic conditions such as diabetes, hypertension, kidney disease, and other chronic medical conditions
- Treat acute conditions such as minor injuries and other acute medical conditions such as infections or exacerbations of chronic diseases
- Facilitates patient and family decision-making regarding treatment decisions, end-of-life care, and severe illness discussions
- Prescribes pharmacological and non-pharmacological interventions
- Our Client is a Medicare Advantage Plan that operates as a health system focused on Quality/Value-Based Outcomes
- Their care model limits daily volume to allow our providers the time needed to provide quality care to their patients
- They offer clinical and leadership growth and stability second to none
- Providers are supported to practice at the peak of their license

Preferred Qualifications:
- Home health experience or outpatient clinic experience treating senior patients
- A working understanding and support of a managed care environment
